Step #1: Complete Intake Forms

Once you have activated and logged into your Burke Care Portal, you will be prompted to complete intake forms. Please complete these forms prior to your first session.

*All sessions are currently held virtually. Someone from Burke Care will contact you with instructions to join the session.*

Step #2: Schedule Appointment

Using the Burke Care Portal select the individual you would like to meet with and schedule a time to meet

If you can not locate a day or time that works with your schedule, please email the individual to learn more about their schedule ask any follow up questions.
